The Pathways palliative care program will present a free educational session from 6:30 - 7:30 p.m. Tuesday at the Lockport Presbyterian Home, 327 High St.
Pathways provides case management support and services to Niagara County residents dealing with serious, chronic and progressive illness. Established in March 2013, the program aims to improve quality of life through symptom management and assistance navigating the health care system. Support from Pathways also helps to reduce the need for emergency room visits and hospitalizations, while keeping individuals comfortable in their own home, adult home or assisted living facility.
“Each Pathways patient’s individual needs are assessed and valued," said Pathways Director Joann Stoll. "Pathways staff work with primary care physicians to give seriously ill patients the best option to live life more comfortably, to breathe easier and to receive optimal pain management.”
Stoll says Advance care planning is also an integral part of the service that helps patients understand and complete Advance Directives such as a Health Care Proxy and Living Will.
Lockport Presbyterian Home Administrator Colleen Bullion said: “We look forward to hosting the Pathways program and offering education to the communities we serve. Learning about available options for comfort care before you need them is vital in making ideal choices when our health becomes compromised.”
The program will address the differences between palliative care and hospice care, when is the right time to call for palliative or hospice care, as well as provide an opportunity for questions from the community to be answered. Refreshments will be provided. Following the program, tours of the beautiful and historic Lockport Presbyterian Home will also be offered. No prior registration is required.
