Kalos Health (formerly First Choice Health), a Managed Long Term Care (MLTC) program providing care coordination for Medicaid recipients diagnosed with a chronic illness, has applied to the New York State Department of Health (NYSDOH) to expand services from Erie and Niagara Counties to Orleans, Genesee and Chautauqua Counties.

Kalos Health provides individuals 18 and older with personalized care management services with the goal of helping members remain in their own home. Care Managers work with the member and a team of health care professionals to develop a comprehensive plan to ensure the member’s health and wellness needs are met.

Since its inception in 2014, Kalos Health has grown to be the largest MLTC program in WNY.

John Lomeo, President and CEO said, “A need for expansion into rural areas of Western New York was identified by the NYSDOH based upon the lack of service and providers in those areas.” Expansion plans have been assisted by a $200,000 NYSDOH Balanced Incentive Program grant that was recently awarded to Kalos Health.

Kalos Health currently has offices in Lockport, Niagara Falls and at Erie County Medical Center (ECMC) with plans for additional locations in the expanded rural areas. For more information call 1-800-894-2464 or visit www.KalosHealth.org.
